Records are shared only with authorised recipients and only to the extent reasonably required for the agreed purpose.

- The contracting legal entity
- The information requested and why
- Where it will be processed or stored
- Which providers or processors may receive it
- Retention, access and deletion rights
- How to withdraw or change authorisation
Patient choices
Access, correction, withdrawal and re-authorisation.
Subject to applicable law and clinical record obligations, patients may ask what information is held, request correction, withdraw future optional sharing, or define a new scope and expiry for access. Withdrawal cannot reverse lawful processing or copies already made by authorised medical recipients.
